Is the return monthly or yearly?

You choose. When you select "per year", we convert it to the equivalent monthly rate with the compound formula (1+i)^(1/12)−1, so the month-by-month simulation is correct.

Should I use a real or nominal return?

For today’s purchasing power, use the real return (net of inflation). For the face value in the future, use the nominal one. R$ 1 million in 30 years is worth far less than R$ 1 million today.

Does it account for income tax?

No — the result is gross. Fixed-income investments usually have 15%–22.5% tax on earnings; for a net view, reduce the rate proportionally.
